Virtual and phone appointments:

1) Call to determine if telemedicine services are necessary

2) Schedule a video or phone appointment
​
3) Recieve a text message with a link to a UPDOX (HIPAA compliant) video chat if video appointment is selected
​
4) If new patient, fill out the forms below before visit
If Dr. Nair determines an in-person examination is necessary after screening:
​
1) Exam rooms are thoroughly sanitized and all staff wear PPE protection
​
2) On arrival, please wear a mask and call (661) 633-1983 to check in from the vehicle
​
3) Patients are brought into the office one at a time
​
4) If you have COVID-19 symptoms, have pending results, or were in recent contact with someone who tested positive, please call to reschedule
> If you are a new patient, print and fill out these forms, then fax or bring them into the office
​
> If you do not have a printer accessible, you can fill out the forms in the office
